Identify the page and exact claim, explain the correction, and provide the strongest available source with a timestamp, page, chapter, mission, collectible or game-build locator.

Conflicting evidence is recorded as disputed until it can be responsibly resolved. Reports are not treated as verified merely because they are plausible or widely repeated.
